Patient Information

If you are a new patient to our clinic, please arrive at least five minutes before your consultation or fill out a new patient form here and bring the following to your consultation:

  • Photo ID
  • A valid Medicare card
  • Workcover patients will need to inform their employer and bring their employer’s contact details for verification to arrange payment.

Fees

Billing Policy

We are a bulk billing practice. Most consultations are time based and will incur private fees for patients who are not eligible under the bulk billing criteria.

Fees are payable at the time of consultation by cash, credit card or eftpos.

We are still able to bulk bill eligible patient groups for most of the consultation types:

Bulk Billing is available for:

  • ALL PATIENTS
  • Children 15 years and under
  • DVA Gold Card Holders
  • Pensioner concession card holders 65 years and over
  • Cervical screenings
  • Skin cancer checks
  • Procedures
  • Transfer of History

Services excluded from bulk billing include:

  • WorkCover
  • RHCA Medicare cardholders
  • Pre-employment medical assessments
  • Medical/other reports
  • Drive Medical Assessment
    (Taxi/Uber/Diabetic/Racing)
  • Cosmetic Treatment’s
  • Urine Drug screening
  • Allied Health services

WorkCover and TAC Services

Current policy regarding WorkCover and Transport Accident Commission (TAC) consultations:

  1. Upfront Payment Required: All WorkCover or TAC consultations require full payment on the day of service.
  2. Reimbursement Process: Following payment, a tax invoice and receipt will be provided for patients to submit to WorkCover or TAC for reimbursement.
  3. Patient Responsibility: Direct billing to insurers is not available.

Patients attending under WorkCover must inform their employer prior to the visit and bring their employer’s contact details for verification.

Contacting Your Health Professional

Phone calls to your doctor will be put through initially to the nurse if available or the nurse will return your call as soon as possible. The nurse will then discuss any concerns with your doctor and get him/her to phone you back as required. If your doctor has asked you to phone, please advise reception and you will be put through or a message will be taken for the Doctor to return your call as soon as possible.

Reminder System

Our practice is committed to providing continuing care and preventative care. We operate a reminder system for preventative health services. Please make sure your contact details are kept up-to-date.

Delays

We hope you understand that delays sometimes occur due to unforeseen circumstances which are out of our control. We endeavour to see patients as close to their appointment time as possible.

Results

A follow up appointment is required to receive results. Telephone advice will only be
made if authorised by your doctor. Mambourin Doctors will attempt to contact you regarding any urgent results.

Prescriptions

Check your medications regularly and ensure you have sufficient supply until your next appointment. An appointment is required for all repeat prescriptions.

There is Parking onsite around the building and in surrounding streets.

Booking Appointments

Appointments may be booked via online (Hotdoc) or by calling: (03) 8731 1633 in person Monday to Friday 9:00 AM – 5:00 PM. Regular appointment times are 15 minutes, if you feel you need longer, please discuss with the receptionist at time of booking.

No-shows, late arrivals (>5 mins), and late cancellations incur $30 fee.

Compliments and Complaints

If at any time you have a concern, comment, or suggestion, please do not hesitate to fill in a Compliment, Comment, or Complaint form available in the Mambourin Doctors waiting room. MyMedicare

We invite all existing and new patients who consider us to be your preferred practice, to register for MyMedicare. To complete a registration, scan the QR code or register using the link provided on this page. If you have any questions, please contact our team.

What is MyMedicare?

The Australian Government has introduced a new voluntary patient registration model called MyMedicare to drive improvements in primary health care for all Australians and deliver new funding packages to primary care providers. MyMedicare will strengthen the relationship between patients and their health care teams by allowing patients to register with their regular general practice and select their preferred GP and providers within the practice. What are the benefits of MyMedicare for our patients?

The key benefits to you include better access, better continuity of care, and better planning. Registration in MyMedicare is not compulsory, however it is recommended to improve continuity of care for patients. You can also change your mind and leave the program at any time.
